Illinois Fire Department Tax Filing -
with the Illinois Municipal League
All filings are due by: August 29, 2008
Process for electronic filing:
1) Review the Instructions, completely as changes have been made from 2007.
2) Review the Bulletin, as some code numbers have changed from 2007 and there are new city/village/FPD’s filing for the first time in 2007. The complete list of codes is available in the 2008 Illinois Fire Department Tax Code Numbers book.
3) Complete the Excel spreadsheet schedule provided by the IML or submit another acceptable electronic filing approved by the IML. To file electronically in another format please contact Craig Elliott at 217/525-1220.
· DO NOT put multiple worksheets in an electronic filing. Please submit a separate Excel document for each insurance company.
· When complete, email your attached schedule to firetax@iml.org, with "Submit" and your Insurance Company Code Number in the subject line.
4) Complete the Sworn Affidavit Signature Page and mail it along with your check payable to the "Illinois Municipal League Fire Tax Fund", for the total tax due to the address below. The total amounts listed on this page should match your totals on the Excel schedule.
5) After the IML has completely processed the Schedule, Sworn Affidavit Signature Page, and check an acknowledgment letter will be mailed to you as confirmation. All amounts of tax submitted to the IML by the insurance companies will be submitted to the Illinois Department of Financial and Professional Regulation – Division of Insurance.

Important filing facts:
* All code numbers must match our records. Please update your codes by using the 2008 Illinois Fire Department Tax Code Numbers Book. Schedules submitted with incorrect code numbers will be returned for correction.
* No credits are allowed to be submitted on the schedules. Schedules submitted with credits will be returned for correction.
* The Illinois Municipal League does not collect for the following municipalities, please pay them directly and file a record of those payments with the Illinois Department of Financial and Professional Regulation – Division of Insurance, 320 West Washington, Springfield, Illinois 62767.
